  If you are a candidate on an eligible list, to confirm your address, you must provide three (3) documents from the list below. The standard for proof is domicile (your primary place of residence).

  Accepted Documents

  • Valid driver’s license (current address; P.O. boxes not accepted)
  • Vehicle registration
  • Property tax receipt
  • Address used on state or federal tax returns
  • Lease agreement or mortgage statement
  • Utility bill (gas, electric, or water) dated within 30 days
  • Bank or credit card statement dated within 30 days
  • Insurance policy or bill showing your address
  Note: Depending on the circumstances, more than 3 documents of proof may be required.

• If you have applied for one or more examinations offered by the County of Rockland, New York State, or other County Civil Commissions on the same date, you must complete this form and file it at the County of Rockland, Department of Personnel, no later than two weeks prior to the exam date.

• Request for Reinstatement to Eligible List
  If you have received notification that you were removed from an eligible list, you may request to be reinstated to the eligible list by completing the attached form. Your request to be reinstated must include an explanation of the circumstances that resulted in your removal from the list for reasons such as an affirmative declination to the position on a prior canvass, failure to reply to a canvass, failure to appear for an interview or to accept appointment or to report to work. Supporting documentation may be attached as well. Pursuant to Rockland Count Civil Service Rule XI.3, all requests to be reinstated are made at the discretion of the Commissioner of Personnel
