What Are Long-Term Services and Supports?
Long-Term Services and Supports (LTSS), sometimes called long-term care, help people of all ages live safely and independently for as long as possible. These services may be medical or non-medical and support daily activities, health, and well-being.
Services may be provided:
- At home
- In the community (such as senior centers, adult day programs, or community centers)
- In residential settings, including assisted living residences
What Is NY Connects?
Finding the right services and supports can be confusing.
NY Connects is a trusted, no cost, and non-biased resource that provides information and assistance about long-term services and supports available in your community.
NY Connects helps individuals understand their options, whether services are paid for privately, through insurance, or through public programs. NY Connects does not promote or sell services and provides information that supports informed decision-making.
NY Connects is part of New York State’s No Wrong Door (NWD) System, which means individuals can enter the system at any point and be connected to the appropriate services and supports, regardless of age, disability, or source of payment.
NY Connects also maintains an online Resource Directory that includes up-to-date listings of long-term services and supports, providers, and community resources available statewide and locally.
