Help Shape the Future of Health in Rockland County

 

 


NEW CITY, NY —
The Rockland County Department of Health (RCDOH) encourages all residents to help identify the health issues that matter most to you and your community by taking a short (just 10–15 minutes), anonymous Community Health Assessment Survey. The deadline for submitting your survey is July 31, 2025. 

You can complete the survey: 

Online (in English, Creole, or Spanish): Visit the Community Health Assessment Survey 

OR 

In-Person (surveys are available in English, Spanish, Creole, Yiddish, and Hebrew):

You can pick up and return a survey at any of the following locations near you during their regular business hours:

  • Blauvelt Free Library, 541 Western Highway, Blauvelt, NY 10913
  • Clarkstown Town Hall (adjacent to the Town Clerk's office) 10 Maple Avenue, New City, NY 10956
  • Finkelstein Memorial Library, 24 Chestnut Street, Spring Valley, NY 10977
  • Church of St. Joseph and St. Boniface, 245 North Main Street, Spring Valley, NY 10977
  • Rockland County WIC, 14 South Main Street, Spring Valley, NY 10977
  • The Haverstraw Center, 50 West Broad Street, Haverstraw, NY 10927
  • Haverstraw King's Daughters Public Library, 10 West Ramapo Road, Garnerville, NY 10923 

Why participate?

  • Make your voice heard on key health concerns affecting your neighborhood.
  • Contribute to the 2025–2030 Community Health Assessment, which shapes local health priorities.
  • Enter to win one of several $50 gift cards (optional).
