Residents Encouraged To Take Short RCDOH Survey To Improve Local Health Services
Participants Have a Chance to Win a $50 Gift Card

NEW CITY, NY, - The Rockland County Department of Health (RCDOH) is inviting residents to take an anonymous 10 to 15 minute Community Health Assessment Survey to help RCDOH better understand the health needs that matter most to residents. The information gathered will guide RCDOH efforts, along with those of local hospitals and organizations, to make Rockland County a healthier place for everyone.
Participants who complete the short survey will have an option to be entered into a sweepstakes for a chance to win one of several $50 gift cards.
Feedback will be utilized to develop the following public health tools:
- Community Health Assessment (CHA): uses public health data, community surveys, and community stakeholder input to identify the county’s health needs.
- Community Health Improvement Plan (CHIP): a long-term strategy that agencies will use to address the county’s public health needs identified in the CHA.
